NTSB releases report following May 2025 SEPTA train crash that injured 10

The National Transportation Safety Board (NTSB) shared its report following a May 2025 SEPTA train crash that left nine passengers and the operator injured.

The incident unfolded on May 11, 2025, around 9:45 a.m., when a northbound SEPTA light rail vehicle operating on the M Line, collided with an end-of-track bumping post on track 1 at the Norristown Transportation
Center Station in Norristown, pennsylvania

There were 15 passengers and 1 operator on board at the time of the accident, officials said. Nine passengers and the operator sustained minor injuries; two passengers were seriously injured.

After the incident, SEPTA estimated the damage to equipment to be about $920,000.

NTSB shared that the probable cause of the collision was “the single point of failure created by SEPTA’s train control system’s design and configuration, which relied on operator engagement to stop trains at platforms and signals.”

Officials reported that the operator of the light rail vehicle had become distracted from his duties for unknown reasons and failed to hit the vehicle’s brakes.

As a result, the automatic train control system was unable to detect the imminent collision and intervene.

Officials said the collision could have been prevented by a train control system that did not depend solely on operator vigilance.

“SEPTA has worked closely with the NTSB and PennDOT throughout this investigation. We have taken a number of corrective actions aimed at preventing this type of incident from occurring again, and will continue to implement additional safety enhancements moving forward.”
